10 Signs to know that Death Is Near
No one can predict the moment of death. But physicians and nurses involved in end-of-life care know that certain symptoms are usually associated with the body's shutting down.
These signs of approaching death are specific to the natural dying process (apart from the effects of particular illnesses the person may have). Not all dying symptoms show up in every person, but most people experience some combination of the following in the final days or hours: continue for signs
1. Loss of appetite Energy needs decline. The person may begin to resist or refuse meals and liquids, or accept only small amounts of bland foods (such as hot cereals). Meat, which is hard to digest, may be refused first. Even favorite foods hold little appeal.
Near the very end of life, the dying person may be physically unable to swallow. How to respond: Don't force-feed; follow the person's cues even though you may be distressed by a loss of interest in eating. Periodically offer ice chips, a popsicle, or sips of water.
Use a moistened warm cloth around the mouth and apply balm to the lips to keep them moist and comfortable.
2. Excessive fatigue and sleep The person may begin to sleep the majority of the day and night as metabolism slows and the decline in food and water contribute to dehydration. He or she becomes difficult to rouse from sleep. The fatigue is so pronounced that awareness of immediate surroundings begins to drift.
How to respond: Permit sleep. Avoid jostling the person awake. Assume that everything you say can be heard, as the sense of hearing is thought to persist, even when the person is unconscious, in a coma, or otherwise not responsive.
3. Increased physical weakness A decline in food intake and lack of energy leads to less energy, even for activities like lifting one's head or shifting in bed. The person may even have difficulty sipping from a straw. How to respond: Focus on keeping the person comfortable.
4. Mental confusion or disorientation Organs begin to fail, including the brain. Higher-order consciousness tends to change. "Few conditions leave people hyperaware when they're dying," says palliative-care physician Ira Byock, author of Dying Well.
The person may not be aware of where he or she is or who else is in the room, may speak or reply less often, may respond to people who can't be seen in the room by others (see Passing Away: What to Expect When Witnessing a Loved One's Death), may seem to say nonsensical things, may be confused about time, or may act restless and pick at bed linens. How to respond: Remain calm and reassuring. Speak to the person softly, and identify yourself when you approach.
5. Labored breathing Breath intakes and exhales become raggedy, irregular, and labored. A distinctive pattern called Cheyne-Stokes respiration might be heard: a loud, deep inhalation is followed by a pause of not breathing (apnea) for between five seconds to as long as a full minute, before a loud, deep breath resumes and again slowly peters out. Sometimes excessive secretions create loud, gurling inhalations and exhalations that some people call a "death rattle.
" How to respond: The stopped breathing or loud rattle can be alarming to listeners, but the dying person is unaware of this changed breathing; focus on overall comfort. Positions that may help: the head slightly elevated with a pillow, sitting up well-supported, or the head or lying body tilted to the side slightly. Moisten the mouth with a wet cloth and moisturize with lip balm or petroleum jelly.
6. Social withdrawal As the body shuts down, the dying person may gradually lose interest in those nearby. He or she may stop talking or mutter unintelligibly, stop responding to questions, or simply turn away. A few days before receding socially for the last time, the dying person sometimes surprises loved ones with an unexpected burst of alert, attentive behavior.
This can last less than an hour or up to a full day. How to respond: Be aware that this is a natural part of the dying process and not a reflection of your relationship. Maintain a physical presence by touching the dying person and continuing to talk, if it feels appropriate, without demanding anything back. Treasure an alert interlude if and when it occurs, because it's almost always fleeting.
7. Changes in urination Little going in (as the person loses interest in food and drink) means little coming out. Dropping blood pressure, part of the dying process (and therefore not treated at this point, in tandem with other symptoms), also contributes to the kidneys shutting down. The concentrated urine is brownish, reddish, or tea-colored. Loss of bladder and bowel control may happen late in the dying process. How to respond: Hospice medical staff sometimes decides that a catheter is necessary, although not in the final hours of life. Kidney failure can increase blood toxins and contribute to a peaceful coma before death. Add a bed pad when placing fresh sheets.
8. Swelling in the feet and ankles As the kidneys are less able to process bodily fluids, they can accumulate and get deposited in areas of the body away from the heart, in the feet and ankles especially. These places, and sometimes also the hands, face, or feet, take on a swollen, puffy appearance. How to respond: Usually no special treatment (such as diuretics) is given when the swelling seems directly related to the dying process. (The swelling is the result of the natural death process, not its cause.)
9. Coolness in the tips of the fingers and toes In the hours or minutes before death, blood circulation draws back from the periphery of the body to help the vital organs. As this happens, the extremities (hands, feet, fingers, toes) become notably cooler. Nail beds may also look more pale, or bluish. How to respond: A warm blanket can keep the person comfortable, or he or she may be oblivious. The person may complain about the weight of coverings on the legs, so keep them loose.
10. Mottled veins Skin that had been uniformly pale or ashen develops a distinctive pattern of purplish/reddish/bluish mottling as one of the later signs of death approaching. This is the result of reduced blood circulation. It may be seen first on the soles of the feet. How to respond: No special steps need to be taken. Note: These general signs of impending death can vary in sequence and combination from person to person. If a person is on life support (respirator, feeding tube), the process dying follows can be different.

Congolese Model Noëlla Musunka becomes 2013 AFW London Amb.
Africa
Fashion Week London (AFWL) organisers are delighted to welcome model and
philantrophist Noëlla Coursaris Musunka who joins fellow ambassador,
Ghanaian model and philanthropist Menaye Donkor Muntari, and champion of
African fashion, as a AFWL 2013 Ambassador for the third edition of the fashion
event taking place from 1st to 3rd August, 2013.
Africa
Fashion Week London 2013 will see over 100 African designers and exhibitors
showcasing their designs at the event, taking place in the impressive Truman
Brewery space, just off Brick Lane. This years’ AFWL is set to be bigger and
better than ever, and will be the largest fashion event in Europe dedicated to
African and African-inspired fashion. Organisers are expecting over 25,000
people to attend the event over the course of the three days.
Noëlla Coursaris Musunka is the founder of
Georges Malaika Foundation as well as an internationally renowned model who has
graced the pages of a number of fashion magazines such as Vanity Fair, Arena
Magazine, NewAfricanWoman, Arise
Magazine and led a number of other fashion campaigns such as that of Agent
Provocateur.
Born in Lubumbashi, Democratic Republic of the
Congo and raised in Belgium and Switzerland, after achieving a degree in
business management, Noëlla moved to London and embarked on a career in
international modelling. Her successful modelling career brought Noella to New
York where in 2007, in tandem with her active modeling career, she started the
Georges Malaika Foundation, a non-profit organization founded by Noëlla, which
is dedicated to providing all the tools necessary for young girls and
communities in the Democratic Republic of the Congo to unlock their potential
through education.
A fierce activist who works towards the
empowerment of African girls and women, Noëlla has spoken at the UN and at the Kinshasa Parliament, and has had
her work featured on the BBC’s Focus Africa. She spoke during the Opening
Plenary of the 2012 Clinton Global Initiative Annual Meeting and she hosted the
2013 Cinema for Peace Gala for Humanity, where Ben Affleck received an award
for his work in DR Congo.
On her
appointment as AFWL Ambassador 2013, Noëlla said: "I am honored to be an
ambassador for AFWL 2013 as I strongly believe in the power of fashion to
inspire and connect people. I am delighted to be representing African designers
and to help them showcase their talents for the world."
Named as a “major modern-day activist” by CNN in 2010, Noëlla attended the Clinton Global Initiative Annual Meeting 2012
addressing President Bill Clinton during the Opening Plenary, highlighting
GMF’s work in the Congo. She also hosted Cinema for Peace New York 2013 in Los
Angeles where Ben Affleck was awarded for his work in the Congo. In January
2013, Noëlla received one of the Diamond Empowerment Fund’s GOOD Awards honoring
her for outstanding leadership and for GMF’s positive impact in the DRC.

Doctor mutilates baby’s corpse, removes eyes, tongue,private part
The Ogun State Police Command, Sunday, said it has arrested one Dr. Babawale Joshua who owned Ajike Medical Centre at 11, Adeyemi Street, Saka, Sango Ota, for suspected ritual activities and disrespect to the corpse of a 13-month-old baby who died in his hospital, Saturday.
According the Police Public Relations Officer, PPRO, for the state, Mr Muyiwa Ogunjobi, the police discovered that the boy’s corpse was mutilated. He told Vanguard that the eyes, tongue, two lips, two ears, private part, nails and the left hand and veins were cut off at the hospital.Father’s account
In his account of what happened, the child’s father, identified as Obinna Oleh, a 35-year-old auto spare part dealer at Ladipo market, from Umunze village in AnambraState, said: “On Friday the 7th of June, my child took ill, my wife joined me to rush him to the hospital.
“The next day, being Saturday, I came to see them at the hospital in the morning before going to my shop while the child was actually getting better then.
“It was with this hope that I left for my shop thinking that at least before the end of the day, the child would have recovered fully.
“But in the afternoon, my wife called to inform me that the baby’s health had deteriorated, that I should rush home. When I got here, I met my child in a very critical condition.
“I asked if any medication had been administered on him and my wife told me that the doctor gave him an injection that afternoon and that the child’s stomach swelled after the injection.
“The baby was practically abandoned by the hospital management in this condition until around 8p.m when he finally died.
“As we attempted to carry the corpse, the nurse in charge said we had to clear our bills before we could take the body away. She also said she needed to call the Medical Director for the full amount we were to balance before they could release the corpse to us.
“Mind you, we were not owing them before the baby died. We made some initial deposit prior to the baby’s admission and treatment. So when the nurse came up with the balance payment issue, I was not in the mood to argue that with them. I took my wife and we left, with the intention of returning yesterday morning to clear what ever bill there was, and take our child for burial.
“Also our pastor advised us to get a death certificate first before taking the corpse away for burial, so that we don’t encounter any problems with the police.
“We also needed to inform our kinsmen here about what happened. These were the reasons why we left the body of our baby at the hospital till the following morning.
“But yesterday morning, I was having a meeting with my brothers on how to get a doctor’s report on my child’s death when the nurse called me to come over to the hospital, that it’s like they used a spiritual arrow to kill my child. I asked her what she meant by that. And at same time, followed her to the room where the body was.
“I was shocked to the bones to find my child’s body in a gory state. His ears were cut off, his private part, his tongue, his lips, and his eyes were plucked out. I am yet to recover from the shock of that sight.The police was informed and they came and took the body away.”
Meantime, when Vanguard visited the hospital, yesterday, it was under lock and key, having been shut by the police who confirmed the development.

ABUJA — The Presidency, yesterday, reacted to the assertion by former
chairman of the Economic and Financial Crimes Commission, EFCC, Mallam
Nuhu Ribadu that Nigeria under President Goodluck Jonathan is a ‘sinking
ship’.
It said that Ribadu is an ingrate considering what the Jonathan administration did for him on his return from self-imposed exile.
Mallam Ribadu at a lecture in Kaduna on Saturday said Nigeria, under President Goodluck Jonathan, was a sinking ship in which the yearnings of the masses were being neglected by a tyrannical leadership.
Reacting to Ribadu’s claim, the Presidency, in a statement, yesterday, by the Special Adviser to the president on Media and Publicity, Reuben Abati, said in his quest for relevance, Ribadu has resorted to disparaging leaders of the land to satisfy his new leaders.
According to the statement: “The Presidency totally rejects the false, hypocritical and self-serving claim by Mallam Nuhu Ribadu at a lecture in Kaduna on Saturday that Nigeria under President Goodluck Jonathan is a “sinking ship” in which the yearnings of the masses are being neglected by a tyrannical leadership.
“We find it very sad and utterly deplorable that Nuhu Ribadu has resorted to shameless wolf-crying, the peddling of arrant falsehood and the denigration of the elected government of his fatherland in furtherance of his selfish quest for continued national political relevance after his wholesale rejection by Nigerian voters in 2011.
“It is very unfortunate indeed that the once highly respected former EFCC Chairman has now taken to political prostitution and developed a penchant for irresponsible and reckless utterances aimed at improving the electoral fortunes of his new friends and “leader”, who he once famously denounced as ‘not fit to hold public office’.
“There can be no doubt that nothing else but blind ambition for an office for which he is clearly unfit is driving Ribadu to infer that an administration led by a President who welcomed him back to the country after his self-imposed exile, restored his rank in the Nigeria Police to save him from the shame of demotion and converted his dismissal from service to retirement has now become tyrannical and anti-people. We take special note of his ingratitude.

Can there be a greater tyranny than the tyranny of removing governors via undemocratic means and barring legally entitled persons from contesting elections?
“Nothing else but misguided ambition could have driven Ribadu to urge Nigerian youths to rise up and save the country from an administration which he willingly served recently, but which he now duplicitously and insincerely claims is “imposing private interests on the majority.
“It is certainly the height of hypocrisy for Ribadu who built his entire reputation as an anti-corruption crusader by completely disregarding the rule of law and recklessly trampling on the rights of perceived enemies of the government of the day, to now accuse an administration that has consistently upheld the rule of law and respect for fundamental human rights of being tyrannical.
“It is only a shameless man that will turn around and accept to be the political lackey of a man he once openly accused of corruption at various times between 2004 and 2007. Now that he has been used and abused by the undemocratic overlords that reign over the ACN and fearing that he may soon be dumped now that that vehicle is about to be subsumed into the so called All Progressives Congress, Ribadu is desperately seeking fresh relevance.
“Ribadu’s descent into a moral abyss since leaving the exalted office of EFCC Chairman, his equally ethically-challenged new friends and his willingness to vituperate against any person or institution he perceives as a challenge to the fulfillment of his unattainable ambitions, have clearly exposed him for what he truly is – a thoroughly unprincipled attention-seeker whose entire career in the public service was built on bootlicking and doing the bidding of the powers of the day without a care for legality which should have been his primary concern as an officer of the law.
“President Jonathan and his Administration will not be distracted from the diligent implementation of the agenda for national transformation by the falsehoods and vituperations of Ribadu and his new friends.
“Far from being tyrannical as Ribadu falsely alleged in Kaduna, President Jonathan will, as he has consistently done since assuming office, continue to strengthen institutions of democratic governance in Nigeria, uphold the fundamental human rights of all Nigerians including the youth, and protect their right to elect leaders in free, fair and credible elections,” the statement concluded.

Why Does Peter of P’Square Keep Impregnating Lola Omotayo?
This unjustifiable act has been troubling yours truly for some time now
but Olufamous decided to wait till after Lola’s delivery before I raise
my concerns. The attitude of some male musicians with regards to their
ways with ladies is just not encouraging. These guys keep putting ladies
in family way with impunity.
Our focus, however, is
on Peter Okoye and Lola Omotayo. Note that the lady is Yoruba, before
some folks ‘cash in’ on this and call it another post against Igbo
tribe.
Please, what is wrong with Peter Okoye that he has
decided to keep impregnating Lola without taking her to the altar? Or is
getting a woman pregnant at will his own definition of marriage?
After Lola gave birth to a baby boy for him in September 2008, out of
wedlock, I thought it was a ‘mistake’ and that Peter Okoye would do the
‘right thing’ shortly after. But I was wrong; nothing happened. Rather,
he impregnated her again and, on Tuesday, she gave birth to another
child for him, this time a baby girl.
But my question is: As
what? Is Lola just a ‘baby factory machine’ for Peter Okoye or what?
This was what Tuface Idibia did to several ladies and today he’s
“struggling” to marry one of them.

Peter alone. Lola should appeal to Peter to do the right thing but
recently she proclaimed to the world that she does not give a damn if
Peter marries her or not, that Peter is a generous and great guy and
that she does not feel marriage is that significant. So now who is to
blame?

The trend which is gradually becoming a
permanent fixture in Nigeria, in which Northern states are allotted very
low cut-off marks for entrance into Federal Unity Colleges, while those
in the South are allotted higher cut-off marks continues in 2013, as
the Federal Ministry of Education’s documents on the cut-off marks for
the various reveals.
Whereas, a pupil from Anambra
irrespective of gender is expected to score at least 139 to gain
admission into the schools, a male pupil from Yobe and a female from
Zamfara is required to score only 2.
Students who score above the cut-off are automatically eligible for admission on merit.
The Education ministry advised parents
to check the results of their wards which have been posted on notice
board of all Unity Colleges throughout the Federation.

PORTS SECURITY: US gives Nigeria 90-day ultimatum
LAGOS—THE United States Government, through its Embassy in Lagos, has
cautioned the Federal Government to improve on its ports security system
within 90 days or face the stoppage of sail of vessels to Nigeria. The
Embassy, Vanguard gathered, got an audit report from the officials of
the United States Coast Guards who were in Nigeria
about a month ago, to inspect security of the Nigerian ports. Based on
the report, the embassy sent a diplomatic note to the Foreign Affairs
Ministry in Nigeria, which in turn contacted the Nigerian Ports
Authority (NPA) and other relevant agencies.
Confirming the
development, General Manager in charge of security at the Nigerian Ports
Authority (NPA), Col Jamil Tahir (Rtd) told Vanguard that a Diplomatic
Note was issued to the foreign affairs ministry to protest the poor
state of security at the ports, particularly the Tin-Can Island Port.
Tahir stated that the ports authority has swung into action, following a
stakeholders’ meeting called by NPA to sensitise everybody on the need
to be pro-active on the security challenges currently facing the ports.
He said that the authority has already secured an approval to instal
counter terrorism equipment in strategic areas in and around the port
premises.
The 90-day ultimatum, according to Tahir, started
last month and will expire at the end of August. He said that the US
threatened to stop shipping services to Nigeria and to also mobilize its
allied countries to do the same if the port security situation was not
improved upon. At a Port Facility Security Officers (PFSO) Forum meeting
in Lagos last week, a member of the group who preferred to remain
anonymous said that the situation at the ports after the visit of the
U.S coast guards has not changed as the people still hibernate under
disused vehicles around the ports. It was also said that the
Tin-CanIslandPort in particular is like a regular market place where all
kinds of people have unrestricted access while broken down vehicles
liter the port’s access roads.
Besides the measures being put
in place to secure the nation’s ports by the NPA, the Nigerian Maritime
Administration and Safety Agency (NIMASA) called for a stakeholders’
meeting today (Monday) to intimate stakeholders of the transfer of
Designated Authority (D/A) responsible for the implementation of the
International Ship and Port Facility Security (ISPS) code. The ISPS code
is the International Maritime Organisation (IMO) convention to secure
and safeguard ships and port facilities around the world following the
renewed global terrorism. It will be recalled that the Executive
Director in charge of NIMASA’s Shipping development, Captain Bala Agaba
had said that the ISPS code has not been properly implemented in
Nigeria, adding that with the transfer of Designated Authority to
NIMASA, the industry will begin to see improved ship and port security
in the Nigerian ports.
